Your daughter’s well being and health are an essential piece to her success at Central. It is of the utmost importance that the school has current and accurate health information concerning your child at the beginning of each school year. This information must be submitted no later than AUGUST 1, 2018.

ALL INCOMING FRESHMEN, RISING SENIORS AND TRANSFER STUDENTS must, submit by mail, the additional health forms found here.
ALL STUDENT ATHLETES, interested in trying out for and participating in athletics during the 2018-2019 school year, must review and submit, by mail, the additional forms found here.
PLEASE NOTE: If we do not receive the completed information on time, your daughter will not be allowed to attend classes on the first day of school.
